No fragrances or perfumes please: Many people, including Dr. Jaworski have allergies and asthma, so please refrain from wearing scents or clothing previously worn with perfume when you come in to the office.

Scheduling: We schedule appointments at phone number (978) 921-5885 during office hours. Physical Exams are booked in advance by the receptionist. Realizing illness is unpredictable, we leave a number of same day appointments open for those who have already established care with Dr. Jaworski by first undergoing a complete history and physical. If you are sick you are encouraged to call the office first thing in the morning to ensure a same day appointment. Appointments are scheduled to focus on a particular health concern at a time. If you need to address multiple health problems, please notify the receptionist scheduling your appointment and we will be happy to schedule a series of future appointments for you to ensure that all of your concerns are addressed. For every appointment please bring your insurance office visit co-payment in the form of cash or check only. Please do not ask our office to bill you for this nominal payment which is required through contract by most insurance companies before you are seen. Don’t forget to bring your current medications. Returning patients are expected to complete the questionnaires annually when they have their annual physical exam.

Your first visit must be a full History and Physical. This allows Dr. Jaworski to have a clear understanding of who you are and what ailments your body has had in the past, along with a sense of which illnesses run in your family. When you do become sick, Dr. Jaworski will address your illness in the context of your history and physical. If you have just recently selected Dr. Jaworski as your Primary Care Provider, you should call immediately to schedule your history and physical to establish care with Dr. Jaworski prior to becoming sick. Dr. Jaworski will only see established patients for sick visits. We see only one family member at a time, any additional family members needing attention must have their own appointments scheduled. Since acute illnesses often run in families, we do try to accommodate family members on the same day if we have advance notice.

When you cannot keep an appointment: Please try to notify the office twenty-four hours in advance for a cancellation. When you make the original appointment, we block that time for you. With prior notification, we are able to provide that time for other patients needing appointments. If you do not keep an appointment, and neglect to contact the office, we reserve the right to charge you for that time.
