Office Hours: Monday-Friday 9:00am until 4:30pm

The Northeast PHO: Dr. Jaworski is part of the Northeast Hospital Corporation and is affiliated with Beverly Hospital, Addison Gilbert Hospital in Gloucester, and the Hunt Center in Danvers. By choosing Dr. Jaworski as your Primary Care Physician, you agree to stay within the network for hospital visits and referrals to specialists. Dr. Jaworski and her staff will assist you in choosing your secondary care while considering your preferences.

Evening and Weekend Coverage: When the office is closed, the on-call physician is available for emergencies only and can be reached by calling (978) 887-7736. Phone calls of non-emergency nature such as prescription refill requests and lab/xray results should be made during the next business day. Dr. Jaworski is part of a coverage group of well-respected Family Physicians affiliated with Beverly Hospital.

Prescriptions: When you need a prescription refill call your pharmacy and they can fax a request to our office. To make the process easier, please have the spelling of your medication, dose/strength of medication, and how often you take the medication. Faxes and phone calls for prescription refills are handled during office hours only at phone number (978) 921-5885. Please make every effort if possible to call at least ten days in advance of the first day you need the prescription. This is especially important if the Doctor is out of town for a week at a continuing medical education conference and the covering physician does not have access to your medical record. Prescription refills are usually faxed at the end of the business day to local pharmacies and most pharmacies have prescriptions ready for pickup the next afternoon. Please call your pharmacy the next afternoon after you have placed your request to ensure that your prescription is ready. Some prescription requests will require office visits before being granted to ensure that safe medical practices for prescriptions are being followed. Dr. Jaworski encourages patients to become familiar with the various rates of prescription co-pays in their health insurance plan formulary. Many health insurance plans regularly change the list of medications they will or will not cover and the co-pay you pay for a medication can increase substantially without notice. Some of the mail away prescription plans are more cost effective than using your own insurance plan.

Referrals: Most commercial insurance agencies require a written referral before visiting a Specialist. The process is usually as follows:

1. See Dr. Jaworski for the particular concern.

2. Dr. Jaworski determines if you need to see a specialist. If so, a specialist name and telephone number will be provided to you, along with a form for you to complete.

3. Fax the completed form to: Judy Sollows, Noelle Driscoll
Referral Department
80 Lindall Street, First Floor
Danvers, MA 01923
Fax 978 818-6249

If you do not have access to a fax machine then mail your referral to Judy & Noelle at least 10 days in advance of your specialist appointment.

4. Call the specialist's office 4 days before your appointment to insure that your referral has been processed and approved.

Doctor Jaworski’s tips:

Exercise briskly 150 minutes every week (3 days of 50 minutes, or 5 days of 30 minutes).

Drink extra water when you think you are becoming sick, and remember frequent hand-washing to keep from getting sick or spreading your illness to others during cold and flu season.

Many of the top infectious disease experts stress that most respiratory illnesses are viral and do not require antibiotics. If a respiratory illness lasts longer than fourteen days then it is important to be seen for a possible bacterial infection which would respond to antibiotic therapy.

Chronic respiratory symptoms can be a sign of allergies.

Think positive and don’t forget to laugh!
